Before applying for a practice permit for December 1, 2022, and after, individuals must arrange their own Professional Liability Insurance (PLI).
PLI Requirements
- name of the insured must be the same as the name of the person applying for registration/renewal
- coverage is occurrence-based
- insured for a minimum of at least two-million dollars ($2,000,000.00) per occurrence
- annual aggregate limit is at least three million dollars ($3,000,000.00)
Current Dental Assistants
Dental assistants who apply to renewal their practice permit this fall must upload proof of having PLI coverage that takes effect December 1, 2022.

New Applicants
In November, anyone applying for registration and a practice permit that takes effect December 1, 2022, must upload proof of having PLI coverage that takes effect December 1, 2022.
On or after December 1, 2022, anyone applying for registration and a practice permit must upload proof of having current PLI coverage.

Proof of PLI
Proof of coverage must include:
- PLI policy number
- name of the insured (must be the same as the name of the person applying for registration/renewal)
- address of the insured
- policy period (start and end dates)
- coverage details that meet our PLI requirements (listed above)
These are examples of proof of PLI coverage (if they include the items listed above):
- a copy of the PLI policy certificate
- written confirmation from the insurer
- a completed PLI declaration form (using this Applicant’s Declaration: PLI Coverage form)
- proof of third-party PLI
