If you’ve recently changed your name, moved or changed any of your contact information, you must inform the College.
You can update your name, address, phone number, email address, or any other contact information online by signing in to the portal and updating your Profile.
For name changes, you’ll also need to upload a copy of your legal name document or marriage certificate. Please allow up to five business days for a name change to appear on your profile.

Access your profile, view and print your practice permit, update your information and submit requests to the College.
Renew your permit
You’re responsible for keeping your permit current, renewing it annually and notifying the College and your employer of any changes to your registration status.
